Download, print and fax the Alvin Dealer Information Form to Alvin

The Resale Info Section of this online Dealer Information Form

OR

Download, print and fax the Multi-Jurisdictional Uniform Resale Cerificate to Alvin

OR

Fax copies of your a copy of your RESALE certificate and/or a tax-exempt certificate (not sales tax registration certificate) to Alvin

All faxed documents should be sent to us toll-free at (800) 777-2896. Please mark fax “Attn: Credit Manager”.

To avoid delay, please supply fax numbers and email addresses for all of your references.
